What you'll need:

  • Your youth's taxpayer identification number and one of the following forms of identification:
    • School ID
    • California State I.D. Card or Driver’s License

    • Birth Certificate for minors 0–14. (Minors 15 to 17 years old must pressent a picture I.D. Card.)
